GLP-1 Medications vs Surgery

Weight loss medications are becoming increasingly popular, and you’ve almost surely heard of the benefits of medicines like Wegovy and Zepbound through social media or on local and national news. Many have referred to these medications as a potential fountain of youth when discussing their metabolic and cardiovascular benefits. These drugs can be helpful for certain patients, as losing weight and maintaining a healthy lifestyle can be life-changing. For someone dealing with obesity, these medications offer a new chance at living their best life.

These drugs have many benefits, but are they effective for bariatric patients? How do they compare with weight loss surgeries such as the gastric sleeve?

It is important to understand how weight loss drugs such as Wegovy and Zepbound work at the cellular level. The active ingredient in Wegovy is semaglutide, which mimics a hormone in your body to create a feeling of fullness by sending signals to your brain. In Zepbound, the active ingredient is tirzepatide. It works in a similar way to create a sensation of fullness and to slow the rate at which the stomach empties. The difference between the two medications lies in their mechanism of action at specific hormone receptors: tirzepatide acts on both GLP-1 and GIP receptors, whereas semaglutide acts only on GLP-1 receptors. Wegovy is approved for adults and children aged 12 years or older with obesity. It is an injectable drug administered once a week, starting at a low dose and working up to a maintenance dose of 2.4mg. Recently, a tablet version has been introduced.

 

When Medication Makes Sense

There are several points to consider when discussing Wegovy and Zepbound with your weight-loss specialist. The first discussion involves the expected results and BMI considerations. Clinical trials show that Zepbound can result in an average total body weight loss of about 20-25%. For individuals with a high BMI, this amount of weight loss may not be sufficient to reach all their health goals. Wegovy has a slightly lower average weight loss of around 15-20% of total body weight. The next discussion should be about side effects. Common side effects include nausea, vomiting, headaches, fatigue, and constipation. Rare but serious risks include pancreatitis, gallbladder issues, and potential thyroid tumors. Another important consideration is the cost. The drugs can be costly, though recent pricing changes have made them more affordable.

While the appeal of injectable weight-loss drugs like Wegovy and Zepbound is undoubtedly high, they are not necessarily the best fit for every patient. It is important to discuss all of these factors with us.

The Case for Surgery

The gastric sleeve may be the most effective choice for bariatric patients with a BMI of 40-50, or a BMI of 35 with weight-related health conditions. A patient can lose 60% to 70% of their excess body weight by undergoing this procedure, which typically results in significantly more weight loss than medications alone. For patients with extreme BMIs, the Duodenal Switch or SADI may be the best option.

However, a bariatric patient may be prescribed a weight loss medication like Zepbound or Wegovy in addition to surgery in certain situations. While many patients lose weight successfully in the first year or two after surgery, some may experience weight regain years later. In these cases, our team might suggest the drugs to help restart the weight loss journey.
